Welcome BOPpers. This page has been set up to make it easy for you to access forms we will require you to complete during Covid. As time progresses, it is likely these forms will change and be adapted, based on government guidelines. Please check regularly to keep updated.

We have completed a full risk assessment at the practice, have in place strict hygiene measures and all staff will be wearing PPE.
All clients, both existing and new, will need to complete the Covid Screening risk questionnaire once. This will enable the team to assess if you are in a high, moderate or low risk category. Your therapist will also need to call you 24-hours before your visit to complete further screening to ensure your visit to us is safe at this stage.
If you are a new client, please complete your details as best you can on page 3, print all 4 pages and read the consent forms. Please also bring with you a list of any medication and any test results you have (i.e. scan results, blood tests)

GDPR- Track and Trace- Please note, whilst the governments track and trace scheme is operating, we may be contacted to provide data (names, phone numbers, email addresses) of people, including patients, who we have been in close contact with. In this situation, the Information Commissioner and Government have confirmed that public health interest takes priority over your GDPR protection rights. We are therefore obliged to release this information if asked.
